Death Certificate Information

Overview

Certified copies of death certificates can be obtained for anyone who died in the Town of Manchester, Connecticut or who was living in Manchester at the time of the event from 1823 until present.

Death records from 2022 forward can be requested through any town hall in CT.

If you are looking for the records of someone who has died out of town but within the State of Connecticut, please contact the Town Clerk's office in the town where the death occurred or you may call the State DPH Customer Service Office at (860) 509-7700. If you are looking for information on how to obtain Vital Records from other states, please refer to the link for the National Center for Health Statistics.

Death certificates are open records and can be requested by anyone. For anyone who died on or after July 1, 1997, certified copies that show the social security number of the deceased will be issued to the immediate family members only! If the SSN is not to be redacted, immediate family must provide supporting documentation showing that they are eligible. See request form for requirements.

1. Submit

  • The application, payment and current ID will be completed through the VitalChek link below

2. Fee

  • $20.00 per certified copy
  • An additional $7.00 fee is charged by VitalChek for using this service, and all major credit cards are accepted, including American Express, Discover, MasterCard or Visa.
  • Expedited shipping by UPS is available for an additional fee

3. Town Selection

  • During the Vitalchek application process online, under "select a death certificate" section, make sure the order is being submitted to the Town of Manchester and NOT to Connecticut Dept. of Public Health.

 4. Form & ID

  • The signed Identity Verification form and current ID can be emailed or faxed if there is a problem with uploading the forms into VitalChek.

5. Completion of request

  • Request is processed within in a day or two of receiving it.
  • Certified copy will be mailed if all requirements are met.

  • See application form for who has access to be issued a certified copy that includes the SSN. Other forms of identification may be required.

3. Fee

  • $20.00 per certified copy
  • Bank check or Money Order made payable to Manchester Town Clerk's Office or credit card slip

4. Include Self-addressed stamped envelope 

5. Completion of request:

  • Request is processed within in a day or two of receiving it.
  • Certified copy will be mailed if all requirements are met.

Funeral Homes filing death cert and requiring permits:

  1. If a burial permit is required, please fax (860-647-3029) the completed death cert ahead of time.
  2. Death certificates and permits can be submitted for filing by mail, drop box or in person only during business hours of the Town Hall. Please call when certificates are left in the drop box. Any certificates or permits requested will be returned by mail or we can call for you to pick up. Please indicate your preference when the request is submitted.
  3. Request certified copies of an already filed certified death certificate by mail or drop box. The copies will be returned by mail.
  4. Original correction letters can be submitted by mail, drop box or in person. Fax ahead to start the process of the correction.
